Payment & Financial Policy

At Reclaim & Rise Therapy Collective, we are committed to providing structured, professional, and accessible behavioral health services while maintaining financial transparency and sustainability.
This policy outlines payment expectations, insurance procedures, sliding scale eligibility, and financial responsibilities.
1. Insurance & Benefits
If we are credentialed with your insurance provider:
You are responsible for your copay, deductible, and/or coinsurance as indicated on your insurance card.
Verification of benefits does not guarantee payment by insurance.
Any balance not covered by insurance becomes the client’s responsibility.
If we are not credentialed with your insurance plan:
Services may be provided at self-pay rates.
Clients may choose to submit receipts for out-of-network reimbursement if applicable.
Payment is due at the time of service.
2. Self-Pay Rates
Standard fees are based on service type and CPT/H-Code designation, including but not limited to:
90791 – Intake Evaluation
90832 / 90834 / 90837 – Individual Therapy
90847 / 90846 – Family Therapy
90853 – Group Therapy
90839 – Crisis Services
H0015 – Intensive Outpatient Program (IOP)
H0038 – Peer Recovery Services
T1017 – Case Management
Session length and billing must comply with federal time-based coding requirements.
3. 2026 Sliding Fee Scale Policy
We offer a structured sliding fee scale for uninsured clients or those experiencing financial hardship.
Eligibility is determined by:
Household size
Annual gross income
Federal Poverty Level (FPL) guidelines
Clients are assigned to one of four tiers:
Tier 1 – Nominal Fee (0–100% FPL)
Tier 2 – 60% Discount (101–150% FPL)
Tier 3 – 40% Discount (151–200% FPL)
Tier 4 – Full Fee (Over 200% FPL)
Verification Required for Tiers 1–3:
Most recent tax return, OR
Two recent pay stubs
Sliding scale approval must be reviewed annually.
To ensure sustainability of services, no more than 20% of total client slots may be allocated to discounted tiers at any given time.
4. Payment Expectations
Payment is due at the time of service unless prior arrangements have been approved.
IOP daily rates and group fees are due per attendance.
Missed sessions may be subject to a late cancellation or no-show fee.
Outstanding balances may result in pause of services until resolved.

Debit/Credit Card
HSA/FSA (if applicable)
Approved electronic payment systems
5. Court-Ordered & Referral Programs
For court-ordered services:
Payment arrangements must be established prior to program participation.
Attendance and compliance documentation are provided per signed ROI.
Financial hardship considerations may be reviewed under sliding scale eligibility.
6. Release of Information (ROI) & Confidentiality
Clients may revoke a Release of Information (ROI) at any time in writing unless court-mandated.
When an ROI is active:
Only summary information is shared.
This includes attendance, treatment goals, progress updates, and compliance status.
Detailed session content and personal disclosures are not released.
ROIs expire one year from the date signed unless otherwise specified.
7. Returning Clients
Clients may return to services at any time, subject to availability and updated intake procedures.

We believe financial transparency supports trust and stability. Our sliding scale is designed to increase access while maintaining the structure necessary to provide high-quality, ethical, evidence-based behavioral health services.
