Complete the short form and submit your contact information so we know how to reach you.
Step 2
Check Your Email
Within 24 hours, you will receive an email inviting you to create a password for your client portal. Once logged in, you will complete your intake paperwork.
Step 3
Schedule Your Appointment
Once your paperwork is submitted, someone from our team will contact you to schedule your first appointment. In most cases, once you are registered, you will also have the option to schedule appointments directly through your portal. Either way, we will make sure you are supported through the process.
